• A proforma to for logging your time and activities can be accessed from your branch office page.

• You will need to complete a SPEF-R Self-Assessment across the placement week. The form will be provided to you. However you can view it via this link.

• Your Clinical Supervisor will document your evaluation via a SPEF-R Placement Evaluation form. You can view the form here. 

• You will be asked to use a separate SPEF form to evaluate your placement experience. The form will be provided to you, but you can view it here.

• A complete SPEF-R, including instructions for how to complete it can be found here for reference.

• A how-to guide for the using the SPEF-R as provided by the publishers of the instrument can be found here.
